Emergency Response Information Kit

An Emergency Response Information Kit (E.R.I.K) provides essential information so responders can provide the correct care in an emergency medical situation.
​Once completed, the kit should be placed on your fridge door, where emergency crews have been trained to look for it, ensuring it is readily available if needed.​​

An E.R.I.K folder contains:

Health information form. Your health care professional, pharmacist or family may assist you in filling it out

Space for your health care directive

Information about how to register intent to be a donor using Manitoba’s online organ and tissue donor registry

A sticker for the front entrance of your house which alerts paramedics to look for your E.R.I.K.

A magnet to hold the information packet in place on your refrigerator door
