An Emergency Response Information Kit (E.R.I.K) provides essential information so responders can provide the correct care in an emergency medical situation.

 

​Once completed, the kit should be placed on your fridge door, where emergency crews have been trained to look for it, ensuring it is readily available if needed.​​

​

An E.R.I.K folder contains:

• Health information form. Your health care professional, pharmacist or family may assist you in filling it out

• Space for your health care directive 

• Information about how to register intent to be a donor using Manitoba’s online organ and tissue donor registry

• A sticker for the front entrance of your house which alerts paramedics to look for your E.R.I.K.

• A magnet to hold the information packet in place on your refrigerator door
